Customer Poll popup and redirects-- cant find and remove

I continue to get a popup called Customer-Poll Survey and also Google Survey.  I've been able to eliminate the audio part that included an alarm tone and woman's voice but cant eliminate the popup.  The introduction begins with, "Hi, my name is Lara Grace and we have selected you to...
